STATEMENT OF COUNSELING SERVICES


Please read the following statements carefully so that you will understand the procedures for the counseling session. For simplification the singular is used even when the plural may apply.
  • I understand that CCCS will provide a confidential comprehensive personal money management interview.
  • I understand that services will be provided by and/or reviewed by a Certified Consumer Credit Counselor.
  • I understand that in the event I am dissatisfied, I can utilize the Complaint Resolution Process.
  • I understand that part of CCCS funding comes from voluntary contributions from creditors who participate in the Debt Management Plan (DMP). Since creditors have a financial interest in getting paid, most are willing to make a contribution to help fund CCCS. A creditor's contribution, if any, may by up to 15% of each payment received, and neither the absence nor amount of the contribution will affect CCCS willingness to work with my creditors. My account will always be credited with 100% of the amount paid to creditors through CCCS. Any fees paid to CCCS will not be used to pay my creditors.
  • I understand that CCCS does not settle my debts. I will be paying back the full balances of my accounts and that any accounts that cannot be included on the DMP will be explained to me during my appointment and will not appear on the DMP creditor page for payment.
  • I hold CCCS, its employees, agents, and volunteers harmless from any claim, suit, action or demand of my creditors, myself or any other persons resulting from advice or counseling. This provision shall not apply to actions or claims under the provisions of the United States Bankruptcy Code, 11 U.S.C. Sec. 101 et seq.
  • I will be given a written assessment outlining a suggested client action plan which will be based on the following options:
    • I will handle my financial concerns on my own.
    • I may choose to enroll, if I qualify, in the agency's Debt Management Plan. Under the DMP,  CCCS serves as a neutral third party in negotiating with creditors to liquidate financial obligations. CCCS has no responsibility or obligation for any past, present, or future credit rating I receive. In certain circumstances a DMP may affect my credit rating negatively. In the event that a counselor suggests a DMP, I will receive complete details of the operations, requirements and responsibilities.
    • A counselor may answer questions about bankruptcy, but not give legal advice. If legal advice is needed, I will seek appropriate assistance.
    • I will be referred to the other services of Goodwill Industries of KYOWVA Area, Inc. or any other agency or agencies that may be able to assist me with particular problems that have been identified.
    • Receipt of financial counseling and education services does not guarantee participation in debt management program services.
  • My information may be used for confidential research and/or a neutral third party may contact me to request an evaluation of the agency's services.
